Be sure to fill out each space, if something does not apply, put n/a! Any blanks will cause your form to be denied. Also, if you've never been to jail, but have been given a ticket to go to court for something like resisting arrest without violence. Any crime you were given a ticket for, even driving without a lic.; could show up as an arrest, so even though you've never been to jail. Say yes and explain to be safe.
Per the DOC, when you are done filling out the form, send it directly to the camp / prison where your loved one is; address it to "CLASSIFICATION DEPARTMENT" unless you know who your loved one's classification officer is, then send it directly to that person.
